Application for Enrolment

Please use this form to apply for enrolment at Trinity. A separate enrolment form must be completed for each child. An application fee of $55 per child must be paid when this form is given to the school.

Application for Re-Enrolment

Please use this form to apply to re-enrolment a student who has previously attended Trinity. A separate enrolment form must be completed for each re-enrolling student.

Building & Library Fund Payment Authority

Download this form to make a building fund payment by direct debit or credit card. An annual donation of $200 to either the Building or Library Fund is suggested.

Concession Application

Please complete this form and return it to our Accounts team if you wish to apply for a fee concession for the year. Please note that concession must be applied for each year and need to be submitted by mid-January.

PIP Form

Download a PIP form to record your volunteer hours. PIP forms must be returned to Reception by 30 November each year.

Payment Authority

Use this form to change or set up regular direct debit or automatic credit card payments of school fees.

Student Absence Notification

Please complete this form and return to the student office when students have been absent.

Resale of School Uniforms

The School Shop stocks a range of quality second hand uniform items. If you would like to either donate or sell uniform items please complete the following form and return it with the uniform items to the School Shop or Reception. Please ensure that second hand uniform items are clean and in good order before sending them to the School Shop.

Fee Advice Form

Families with new students must return this form with the acceptance of enrolment.
The school needs to receive this form to arrange fees for new students.

Prohibited Persons Declaration

The prohibited persons declaration form should be filled out by parents and guardians who would like to volunteer to help, in any area, in the school, for the year 2013.

You may have already signed and submitted a form previously, but kindly note that it is necessary for each person volunteering, to sign a declaration form every year.
